An employee who wishes to take maternity leave is required to provide notification to her employer by the end of the 15th week before the expected week of childbirth, or as soon as reasonably practicable if that is not reasonably practicable. The notice must include prescribed information.

The employer could require the employee to notify it of her intended maternity leave start date in writing and, if the employer requests, the employee must produce a MAT B1 form from a registered medical practitioner or registered midwife stating the expected week of childbirth.

The employee's expected week of childbirth will not be confirmed until she receives her MAT B1 form, but you could use this calculator if she has given you unofficial notification of maternity leave to give her an indication of when she must give official notification.
