Notification of maternity leave

An employee who wishes to take maternity leave is required to provide notification to her employer by the end of the 15th week before the expected week of childbirth, or as soon as reasonably practicable if that is not reasonably practicable. The notice must include prescribed information. 

Employee's notice: prescribed information
The fact that the employee is pregnant
The date on which the employee intends to start her ordinary maternity leave
The employee's expected week of childbirth

The employer could require the employee to notify it of her intended maternity leave start date in writing and, if the employer requests, the employee must produce a MAT B1 form from a registered medical practitioner or registered midwife stating the expected week of childbirth.
