Form wording: I authorise the Employer to deduct from my [wages/salary] (including
my final [wages/salary] or any other payment due to me on termination of my
employment) the amounts set out below for the purposes described.
Should any amounts still be outstanding after
deductions have been made from my final wages/salary, I agree to repay the
balance in full over a period of time to be determined by the
Employer. |
