Letter wording:

Pursuant to the Access to Medical Reports Act 1988, section 4(1)(a)

[Name of employee] has indicated to us that he/she intends to request access to your medical report about him/her before it is supplied to us. In order to seek access, we have advised him/her that he/she must communicate with you in writing within 21 days of the above date.
