When to use this model parental leave form

Use this model form if you wish to establish how much parental leave a job applicant or new employee has taken with previous employers to confirm what (if any) entitlement to parental leave is still outstanding.

Form wording

Employees who have children under five years of age are entitled in law to a total of 18 weeks' parental leave in respect of each child once they have gained one year's continuous service with their employer.
