Letter wording: I refer to my letter to you of [date] requesting your consent for the Company to seek a medical report from [your GP/the Company's occupational doctor/an occupational doctor nominated by the Company]. The reason for this request (which was explained to you) was so that we could assess:

  • your current state of health in relation to your ability to perform your job;
  • the likelihood of your being able to resume working in the foreseeable future;
  • what, if any, steps we might be able to take to facilitate your return to work and/or to support you on your return; and
  • what action(s) might now reasonably be open to us regarding your ongoing employment with the Company.
