When to use this model risk assessment for new and expectant mothers form

Use this model form where you have received written notification from an employee that she is pregnant, has given birth in the previous six months, or is breastfeeding, or to comply with the duties to carry out a risk assessment where there are women of child-bearing age in the workforce.
