A separation questionnaire is a self-completion form that employees who have tendered their resignation can be encouraged to fill in.
The questionnaire should be designed to gather information about the reasons for the departing employee's resignation and their view of, for example, pay and benefits, training and development and working conditions at the organisation.
The departing employee might also be asked about team morale and working relationships with their line manager and colleagues.
