When to use this model equal opportunities letter
Use this model letter to seek authorisation from an individual for the disclosure of personal information about him or her that has been requested in, or is needed to reply to, a discrimination questionnaire. The questionnaire procedure is repealed from 6 April 2014, although the procedure remains in force for proceedings that relate to a contravention that occurred before 6 April 2014.
