When to use this model paternity leave form
Use this model form to ensure that the partner of an employee who requests additional paternity leave provides the required "mother declaration" of the employee's eligibility to take additional paternity leave in respect of a birth child and, if applicable, to qualify for additional statutory paternity pay.
