When to use this model equal opportunities questionnaire
Use this questions form to familiarise yourself with the form that an individual who thinks that he or she has been discriminated against before 6 April 2014 because of a protected characteristic under the Equality Act 2010 will send to the employer to gather further information. The questionnaire procedure is repealed from 6 April 2014, although the procedure remains in force for proceedings that relate to a contravention that occurred before 6 April 2014.
