Additional paternity leave: new model forms and letters
New model forms and letters on additional paternity leave include: a form for an employee to request additional paternity leave; certification forms for an employee to confirm eligibility; and a letter responding to an employee who intends to take additional paternity leave.
The right to additional paternity leave is available to parents of children due on or after 3 April 2011.
- Form for an employee to request additional paternity leave in respect of a birth child Use this model form to give employees a means of providing formal notification of their intention to take additional paternity leave in respect of a birth child and, if applicable, to receive additional statutory paternity pay.
- Self-certification form in respect of an employee's eligibility for additional paternity leave and pay in respect of a birth child Use this model self-certification form to ensure that an employee who requests additional paternity leave makes the required "employee declaration" of his or her eligibility to take additional paternity leave in respect of a birth child and, if applicable, to receive additional statutory paternity pay.
- Form for the partner of an employee requesting additional paternity leave to declare the employee’s eligibility in respect of a birth child Use this model form to ensure that the partner of an employee who requests additional paternity leave provides the required "mother declaration" of the employee’s eligibility to take additional paternity leave in respect of a birth child and, if applicable, to qualify for additional statutory paternity pay.
- Letter asking an employee for evidence of eligibility for additional paternity leave in respect of a birth child Use this model letter to respond to an employee’s written request to take a period of additional paternity leave to confirm eligibility for additional paternity leave and pay.
- Letter responding to an employee who has notified an intention to take additional paternity leave Use this model letter to respond to an employee who has notified you of his or her intention to take additional paternity leave.
- Form for an employee to request to withdraw from taking additional paternity leave Use this model form to employees to allow employees to notify their intention to withdraw from taking additional paternity leave.
- Form for an employee to request to vary the dates of additional paternity leave Use this model form to allow employees to vary the start or end dates of additional paternity leave.
